The design — and the threshold that actually matters
TARGET-D is pragmatic, not mechanistic. Enrollees were randomized to either standard care or a titration protocol that tracked 25(OH)D and escalated D3 dosing until participants landed inside the 40–80 ng/mL window. That range is not arbitrary: it sits well above the 20 ng/mL deficiency cutoff most reference labs still use, and it overlaps neatly with the "optimal" zone that high-dose advocates have been selling for over a decade. The endpoint is hard — MACE — not a surrogate lipid or inflammatory marker, not a self-reported wellness score. If the marketing claim "correct your D, save your heart" had a biochemical fingerprint, this is the trial designed to catch it in the act.
Adjacent signal: targeted dosing isn't only a cardiac story
Two other randomized reads landed within the same week, and they sharpen the picture. The practical verdict — for now
Until the TARGET-D outcomes table is in hand, the defensible position for clinicians and policymakers is uncomfortable but honest: population-level fortification and deficiency correction remain the only moves supported by serum epidemiology and trial-grade evidence. Chasing a 40–80 ng/mL target in a post-MI patient is a hypothesis under test, not a protocol to import into a clinic — and certainly not a justification to fortify foods at supraphysiological doses. Watch the MACE result, watch the subgroup splits by baseline 25(OH)D, and resist any supplement brand that cites TARGET-D before the data are out.
